 Embarq Underground Communications Vault
Daniels Parkway and Eagle Ridge Drive
August 11, 2008 - C Shift

Patient was unable to remove self from vault.  Vault atmosphere was determined safe and a
LCEMS medic and confined space technician made entry and initiated treatment.  Patient was fitted into a Class 3 harness.
Ladder 63's ladder was used as an anchor point for a 4:1 hauling system with a belay to safely remove patient from vault.
